If you participated in spring updates given here at the college you must also make sure that the following forms are on file with here in the athletic training department. If you had any medical procedures performed (orthopedic surgery or any other surgery in general) over the summer you must bring documentation from that physician with information and clearance to participate in athletics regarding the procedure.

• Health Insurance Information Form (With a copy of the front and back of insurance card(s))
